EVS Techs are responsible for providing a safe care environment for patients and staff (visitors as appropriate to setting) within their assigned area(s) achieved by demonstrating the principles of cleaning, disinfection and quality assurance toward the reduction of harmful pathogens for the benefit of patients, visitors and staff. While executing the assigned tasks, the EVS Tech will have contact with patients and visitors. Contact may include educational and safety-related and should be appropriate to the specific demographics of patients served in assigned service areas. Responsible for ensuring timely room turns related to patient throughput within the basic principles of environmental hygiene and infection prevention. As a Care Coordinator, you will: - Assist clinical case managers by working with clients, gathering, compiling and coordinating continuing care information - Provide AA/NA contacts in clients home area, set up interviews, arrange transportation and contact agencies with regard to clients continuing care plan - Maintain the continuing care database, with special emphasis on continuous updates of clinical value and work - Work with the admissions office and provide back up for admitting clients, when needed, as well as provide the continuing care chart checks for all patients leaving treatment - Follow up phone calls within 5 days of discharge for WI client facility placements for continuing care services.
